Guidelines
Fees are due on signing day. No refunds will be issued. No insurance will be billed. No claims submitted. Membership is stored in our office. Family members must live in the same household. Membership is valid for one year and must be renewed annually.
Dental Exclusions & Limitations
This program is a discount plan, not a dental insurance plan.
- In conjunction with another dental plan or dental insurance
- For treatment which, in the sole opinion of the doctor, lies outside the realm of their capacity
- For hospitalization or hospital charges
- For costs of dental care covered with automobile or medical insurance
- For services of injuries covered under workers compensation
